From an engineering perspective, how does this happen? There must be failsafes on the train, on top of failsafes in the station, on top of redundant systems. It's not like there's a driver who just didn't take his foot off the gas pedal.

nope, that's called "positive train control" and the feds have been demanding it for decades but it costs money.

yes that is literally what it is like. i haven't looked at this system specifically but often if there are failsafes they are ineffective due to lack of maintenance.

a common failsafe is a metal peg that catches an external brake pedal and puts the train into emergency braking. this is great if your dopey engineer overshot the platform at 5mph, but does gently caress-all if he falls asleep and flies past the platform at 60mph while hitting the "i'm still awake" button in his sleep.

Oh my god, I've seen pallet conveyor systems with more safety precautions than this. Disney World has more stringent standards for their Pirates of the Caribbean ride than the New Jersey Transit Authority has for their critical infrastructure.
