It was made by Devworks Game Technology which is a Brazilian company from what I can tell, and Mr Balls is a Genesis all-in-one exclusive along with a bunch of other lovely games this parachute company made to inflate the pack-in number to 100 lol.

apparently previous versions of the brazilian genesis had ecco the dolphin and streets of rage but they took them out to make room for sonic 3d blast and phantasy star 2 and 3, instead of removing mr balls and the meatloaf game
