They are more like butcher's meat hooks. It's called suspension in the body mod industry. The hanging part isnt exactly painful given the bed of nails theory (more hooks, spread load etc) but the piercing part is pretty awful.

Folks used to use fishing hooks with the barbs ground off, but these days there are smooth hooks specifically made by body jewelry manufactures for the purpose. As for pain, getting the hooks put in when I suspended wasn't substantially more painful that any other piercing I've ever had. The worst part is how little pockets of air form under your skin and have to get massaged out. That poo poo hurts.

The picture does look weird, but all mammal's knees bend the same way, including ours. Cats (and others) just have really long feet and they walk on their toes/balls of their feet.
