Pretty easy to explain. It's because the highest institution of the nation doesn't force them not to be.

Sorry, I meant for my question to be rhetorical. The system works this way because it's been engineered to work this way. To address the problems raised by this vascular surgeon, you'd need to change the culture and rules governing the profession. Until that happens, the system will remain exactly the same.

This might be a consideration if the current system were as efficient as possible. The current system for surgery registrars not getting maternity leave doesn't strike me as the best one, let alone the apparent institutionalised misogyny

It's pretty clear that at the moment, specializations aren't being filled by the best possible candidates, they are being filled by the ones who are either male or willing to give head and be quiet about it. This sort of sexism is diluting the overall medical professiona talent pool, and that will kill a lot more people than enforcing a quota ever will.
