Please note that its real eyes can be seen peaking out through holes on the stomach (because the eyes on the Pikachu head are fake and can't actually see anything) and that its tail is just a tree branch. In Super Mario World there are two different types of bullet bill, the small ones with arms and the giant Banzai Bill with the mouth. I think it's also the one that introduces the torpedo ted which has both arms and a mouth.
