The oxygen/acetylene mixture isn’t quite right and the flame is traveling up the tube. Here’s a demonstration: https://www.youtube.com/watch?v=r5_iucw3hEw AvE actually goes over all of the usual causes. At 4 minutes, he gets it too close to the workpiece and the back pressure causes a flashback. Then he cleans the tip to remove possible restrictions that would upset the mixture.
