Engine oil pressure is used on most boxer-type airplane engines to provide the constant speed prop with the force to hold pitch. When oil pressure goes to zero, the prop goes to flat pitch, which in this case over-speeded the entire assembly. Feathering on those types of props is generally electrical or mechanical, if it's available at all. (Almost always no on a single-engine airplane.)

Those aircraft aren't exactly running anything other than the lightest option.

I googled this incident and found the pilot's account of it. He suffered a broken back and arm, and had compound fractures of both his lower left leg bones, all because bailing out at a few hundred MPH from an airplane which doesn't want to fly any more is hard and dangerous. They also found that his chute had nearly failed on opening.

It's not a mechanical failure per se but I gotta post this somewhere. I ordered one of those little blue ELM327 OBD2 Bluetooth interfaces and the little mini CD it comes with is full of pirated copies of various scantools. The windows apps even have handy keygen.exe files in the same folder. Maybe this isn't the right place for this, but I thought it was fuckin' funny and I wanted to share. I gave .99 cents for it and it took like 3 weeks to arrive from wherever the gently caress in China, but it works.

Have you scanned it with antivirus? That’s what I’m really curious about.

This thread needs more pics of random automotive failures and I found some on my old phone from work that I haven't posted yet: Here's what can happen when the wrong brake pads that almost fit properly within the caliper and support bracket assembly finally become stuck: The culprit? Some time ago, the car had been in a wreck on that side and the body shop that repaired it had installed a used suspension knee from a different model or model year, and it had a different caliper/bracket assembly for a smaller diameter rotor, as well as a different brake pad configuration. The pads had been replaced at least once before with the "wrong" set on that side, but the rotor was clearly much older and pitted with a noticeable lip on its outer edge. It all began to fall into place after comparing the old rotor to the new correct size rotor; and then seeing the existing caliper bracket bottom out into it before the bolt holes got anywhere close to lining up. Not to mention the much tighter and awkward brake pad fit within the support bracket itself. I can't remember what kind of car it was specifically but I want to say it was either a Toyota/Lexus or Nissan/Infiniti of some kind. Either way it was somehow determined that the knuckle and bearing remained the same for either application, so a proper caliper/bracket assembly was eventually installed along with the new correct rotor and pads and all was right again. Here's a front hub assembly on a late 90's Nissan Maxima that came in with the complaint of a random low speed wobble and clunking noise. Not only were all five wheel lugs loose, a much bigger surprise lurked beneath the fading and pitting factory aluminum wheel: Thankfully the rest of the hub bearing and axle shaft were otherwise undamaged and the wheel hub damage was relatively minor. All it needed was a new set of wheel studs and nuts to be safe to drive again, for now at least. And finally: The right rear wheel well of a 2007 Ford Escape 4x4 with a clunking noise in the rear that sure sounded like a loose and/or broken shock mount. Yup, broken shock mount alright! Not the first time I've seen rust in the wheel wells on one of these, but never this far gone. One of the body shops we recommended quoted somewhere around $3000 to cut out the damaged section and weld in a replacement panel from a donor vehicle. It was a bit of a bummer considering the rest of the truck was pretty clean and straight with just under 100k miles on it. Hooray for the rust belt.
