Yeah they don't pump you up on pain meds for the surgery, they sedate you. People who have woken up in the middle of surgery (an absolutely horrifying experience) can have to undergo some pretty intense therapy to deal with the PTSD from the experience. Inter-operative awareness is like a loving nightmare come to life because you can hear, see, and feel exactly what is going on, but cannot move because you are likely paralyzed by the sedatives and muscle relaxants, so you can't even indicate that you are aware and feeling to the OR staff, it's up to the anesthesiologist noticing that your readings aren't right and correcting your anesthesia.

Your body still feels all of that pain, and reacts to it, even if you're not conscious of it.

This is basically it. There are Gas goons who can expound a ton more, I work mainly in ICUs. The meds that are used for general anesthesia, namely propofol, medazolam, fentanyl, they are very good at "turning you off" and making sure you are not recording memories or centrally recognizing that pain is occurring. The hardwiring of the neurons from your skin, to your spine, to your adrenal glands and blood vessels and heart and every mechanical organ are still quite awake and kicking though. As a physician cuts through your skin and opens your pelvis and starts rummaging around to cauterize and clip things that are hosed up, and run your bowel to check every centimeter for leaks... trust me your body is feeling and experiencing a lot of this. It won't remember or ever know, but the anesthesiologist sees it by spikes in your blood pressure and heart rate, increases in your respiratory rate, etc. We see the same in the ICU when a person is ventilated and in a coma but still in so much pain they are overcoming their anesthesia. It thus becomes a balance between dosing the person with more medicines to control these things, while not overdosing them to the point of cardiopulmonary collapse. Anesthesiologists have a helluva job. Surgery on anything is nuts. It is why they make patients go through pretty extensive pre-operative evaluations nowadays and even get cardiac stents placed to prevent heart attacks if need be.

I doubt this guy is going to die. He will likely have a poo poo ton of morbidity from this, but he likely will live in some capacity.

The one thing that will always stick with me from Surgical Tech Clinicals was when we shut down a patient's heart for surgery and then restarted it after we were done. I have seen traumatic amputations, brains, the insides of every part of the body (sometimes my own). But that was the thing that caused me to :stare:

We have a president who, five months into his term, is under investigation for obstructing a probe into how the motherfucking Russians undermined the same electoral system that got him into power. Beyond that, we have the GOP becoming increasingly horrified that there's No Brakes on the Trump Train but unable to do anything but keep pushing the throttle. The Democrats' electoral prospects for 2018 are starting to look somewhat decent, which means they've got a year and a half to rescue defeat from the jaws of victory yet again. At a societal level, we've tribalized ourselves to a degree that no one has a shared sense of reality. We have one side that believes Hillary Clinton was sex trafficking children out of a loving pizza parlor, and the other side fervently hoping for proof that there's clandestine footage of Trump watching Russian hookers piss on a bed Obama slept in. Political violence, though pretty low level at this point, is becoming increasingly normalized. Again, one side marches into battle with its soldiers marching under the banner of a cartoon frog, its troops looking like a mix of bloated Operators and half-assed Spartan warriors, against punks and ninjas. This would be fun as gently caress to watch in Ultimate Epic Battle Simulator, but holy poo poo otherwise.

Yeah there is a huge difference in dosage between propofol to knock a person out for surgery and propofol to achieve electrical silence on an EEG (we have to do this for status epilepticus sometimes). Patients definitely have working brain signals during general anesthesia, its probably just lovely and akin to a lot of static on a TV or those garbled porn channels from the 90's or something.

Yeah there's different grades of inter-operative awareness, almost all the time if it happens, the patient has no idea that it happened after the fact. Sometimes though drug resistance and complications from the condition of the patient combine to make it so the anesthesiologist miscalculates badly enough that the patient is aware, and has recall of events that occurred during the surgery, which pretty much would qualify as enough nightmare fuel for the rest of your loving life depending on the procedure.

The trick is people have always been weird and violent. We did a good job at hiding it as a society. Even when it would rear its ugly head, we would just hand wave it away.

Your coworkers are probably hosed up people. The people on SA are probably hosed up. The trick is not to let other people know how hosed up you are. That is why some groups have rules about not talking politics or religion at work spaces or family gatherings. Facebook/the internet is a mistake though since it lets people come together in ways we are not ready for.

You feel like poo poo has changed as you age since you start to see it more, but it has always been there. We just have more ways of seeing it now but we will still forget it almost as fast as it happens.

I woke up towards the tail end of some gnarly oral surgery when I was in college (which I was IV sedated for because gently caress being awake for 18 fillings and 3 extractions) and that was unpleasant as gently caress. I was numbed up but it was still pretty horrifying. I can't loving imagine what it would be like for that to happen when someone's rummaging around in your abdomen. :stonk:

Regarding the static the most interesting hung to me was that the different types of anesthetic (propofol, volatiles, xenon, etc) actually have differently shaped noise patterns. They all knock out high frequency gamma waves that are correlated with consciousness but down in the 1-10Hz and 10-30Hz they each looked pretty different.

That was almost 10 years ago now though and I don't recall all the details. IIRC for example propfol usually had a little peak in the 10-14Hz range. Of course as you said you could crank it up and get near silence on the eeg. Those are the patients that don't feel quite right for potentially months after surgery. (That's where anesthesia is a bit of an art)

Supposedly it's not uncommon for dental anesthesia to just use the inductive part because they'll be fast enough!

(The anesthesiologist I was doing that qEEG stuff for had some, uh, opinions)
