I was sure the crash footage was a pre-tape since they were avoiding the back of the ambulance for a while but then they showed Roman and the ambulance in the same shot.

I feel like they pre-taped the crash and ran the footage when Roman is looking in the mirror. Then, they roll off the unhurt ambulance and put Roman into the crashed one.
