also, a lot of the last generation or two of Dems have been utterly bought and paid for anyway, and wouldn't dare do anything to disappoint a CEO. but they still justify it to themselves with the same old "oh, the lobbyists would just oppose it and kill it anyway, better to save my political capital and collect their money while I work with them to write a bill they'll allow to pass"

Yeah, as Main Paineframe says, it's largely a sort of learned/inherited "common sense." Most of these people aren't literally thinking "this would be good to do, but I don't want to do it because of my donors"; they just convince themselves that it's either not entirely necessary or that it might make things worse. In general, irrational fear of making things worse is a big part of liberal/centrist ideology, since it's the natural mindset of privileged people (who don't personally have much to gain from policy like UHC). On an emotional level, they feel more concerned at the idea that more "radical" reform might break what (to them) is a working system, so they would rather tweak things in a way that is guaranteed to preserve their current quality of life.
