I mean, some small one is okay (like maybe $5-20), but I think $50-100 for a visit to the doctor is excessive and certainly a large enough expense to dissuade people from going. That's a heck of a lot of money if you're living paycheck to paycheck.

I have trouble thinking there are actually enough hypochondriacs for this to be much of a problem, and if you absolutely had to you could set things up such that going to the doctor/hospital more than X number of times caused a copay to kick in.

Yeah, like this, this is reasonable. Though I think $5 falls in a weird area where it's so low it wouldn't actually dissuade hypochondriacs, making it kind of pointless, even if it isn't that harmful.

Dental health being treated separately from other health issues in terms of insurance has always been bizarre to me. Like, a kind of obvious analogue would be dermatology, in that it's also a field with medical importance but also a bunch of applications unrelated to healthcare. We don't treat dermatology as some separate "unnecessary" type of medical care because that would be really stupid.

You can get dental work done in Mexico for 70% cheaper than what can be typically done in the United States. I'm looking down the barrel of 6k in dental work so I'm looking into getting a passport.
