You can have 2 fouls on the same player if one happens during the play and the other happens when the ball is dead.

This isn't like college where it's reviewable (different thing anyway). A lot of bang-bang shoulder hits look like the helmet got involved. The taunting is what sealed it as a dirty and on-purpose hit.
