It's Bloodborne, Rom the Vacuous Spider is a boss that's a huge warped spider thing that's backed up by a ton of their spider buddies. They also have a bunch of cutesy fanart because they're not actually immediately hostile, when you fall into their arena you can walk right up to them and give em a good look over.

Also the first place in the game you can find a great one's insight, a skull where the insides exploded outwards from seeing something so profound your head popped, an item that gives you a huge chunk of lore, the first time you see it is on the lakeside at bergenworth, standing exactly where rom is reflected in the water directly underneath the false moon. I.e. Rom is a moon bug.

Where did you get she was mentally challenged? Its a theory based on context. In the Japanese version Rom's subtitle isn't vacuous but rather a word that more closely translates to "idiot". Between the two versions one can assume that Rom is meant to be mostly empty and devoid of higher thought, both before and after her transformation. The theory is that the entire reason Rom was able to ascend to a greater form while other humans cannot is because she was mentally challenged. Her empty mind made her a better vessel for the Great Ones, presumably because she doesn't have the higher functioning required the actually contemplate the horrible truths about the universe that got bestowed upon her. Usually when that happens your brain is instantly fried and you explode in a blood cloud (frenzy). Also I think thematically it works better and is more appropriate for the practices of the Choir and Bergynworth. A lot of their aesthetic was inspired by old medical practices that were very unethical and frequently experimented on disabled people without any sort of consent. Experimenting on a mentally challenged person fits in with the Choir's practice of "lets inject moon juice into these children and see what happens". There's also a great deal of irony about Rom being one of the closest "humans" to the Great Ones because she is an invalid. It means that despite their higher intellect, the Choir can't obtain their goals without losing their minds. But like most things in Bloodborne its a theory and not a hard truth. I'd always thought Rom was vacuous because the revelation she had/transformation she underwent just burned her mind out or that she now operates on a higher understanding that's completely alien to us

Or alternatively, the transformation reduced her to a child-like state where upon she'll eventually grow into her new mental capabilities, similar to what may have possibly happened to you in squid baby ending. You never really know with Bloodborne.
