There is a framework, like BoA mentioned and which you should read if you are genuinely curious, to work through the issue and take action as necessary. Now, what happens if the complaint gets ignored? Just like anything else, people have to make it work. You can use "well what if people don't act in good faith" as a criticism of literally anything, up to and including the highest levels of government.
