The problem is that it often is played as "Ew, gaaaaay!" but even without using it in that context you're still trying to offend someone by using homosexuality as a negative trait. someone going out of their way to mock a woman's appearance has a lot of weird cultural baggage to it, even if she is kinda dorky looking. it's sort of like if one of the main attacks against ben carson or marco rubio was that they were lazy or if people were criticizing a jewish politician for being greedy. like there's a really unsavory precedent of women being devalued by ~society~ if they're not conventionally attractive, and insulting her appearance plays into that, whether or not it's someone's intent. and honestly, the "lol putin fucks trump in the butt XD" narrative also falls into that same sort of territory. it's all just a little too similar to regular old bigotry to ever really be in good taste
