Pretty minor OSHA compared to the rest of this thread, but at my work, some intrepid soul disconnected the safety interlock on our trash compactor that prevents it from being operated while the door is open. Since it's one of these,  it's more than capable of several thousand pounds of compressive force. I assume a member of the custodial staff disconnected it so that they could turn on the compactor, then open the door and use the broom handle/section of pipe next to the door and controls to manipulate the trash for more efficient compacting, or something. Since wooden shipping palettes get put in it periodically, all I could think was that some poor soul was going to be impaled by a piece of wood under just the right pressure in just the right place to grenade. I reported it on the company website, and two representatives from the relevant departments were down there in 10 minutes investigating it. Later that day, it was fixed, and a sign affixed to the door that says DO NOT OPERATE WITH DOOR OPEN. A happy ending OSHA story, for once.
