drat, that's Battlefield Vegas. They're known for taking good care of their weapons so I'm surprised that happened.

I had something similar happen at a low budget rental range in Orlando. I rented a rusty AK that had been converted to full auto with registered parts (the full auto position was marked with a smiley face, which I don't think is standard for any military). About halfway through the box of ammo the semi-auto position suddenly became full auto. It wasn't a runaway gun so I shrugged and emptied it, and then when it was empty the bolt handle was stuck forward and the fire selector couldn't be pushed all the way up into the safe position. The best I could do was take the magazine out and bring it back.

When I was in Army basic way back in the dark ages of the 80's I had a M60 get stuck on auto - the sear notch can get worn down over time which sticks the trigger. Can happen even with well maintained firearms since it's difficult to detect. Sometimes poo poo just wears out just enough to cause a problem.

The process to stop a runaway 60 is simple btw - give the ammo belt a quick twist.

this is actually kind of OSHA, because he had spent his whole working life painting boxcars for a major train line in the 50s-60s. at that time painting was done in painting "shacks" that were built over the top of a bit of rail line inside the company's property. the trains would roll through and center one car inside the building at a time, then the painting crew would run inside and do all the work. there's no way to prove the idea that over a decade of inhaling all-weather boxcar paint fumes contributed to his cancer, but everyone drat sure suspected it.

If you want to have some fun just google "(state) train derailment" and you'll find dozens of little reported industrial train disasters. Most news major media outlets only report passenger train derailments.
Then you got the big disasters like Lac-Megantic where 42 people were killed by a tsunami of flames when a train derailed.

The intersection of trains and roads seems to me to be a huge blindspot in the way the railway evaluates risk (speaking about the UK as usual though).

I recently had some involvement in the aftermath of a near miss at a level crossing that could have been absolutely catastrophic. But all the risk rankings came out low, as the level crossing was effectively not calculated as part of the ranking.

The risk ranking score for this type of event , which mandates the level of response, takes into consideration the likelihood of the train crossing a "conflict point" and the severity of it doing so. Level crossings are typically not considered to be conflict points. Instead the position of the next train that could be struck is used for the calculations as a rule.

So in the incident I was involved with the conflict point was calculated to be over a kilometre away, making the incident relatively low risk. On paper.
