One time a medevac accidentally dropped one of our (dead) guys into a canyon and I guess I'm wondering how bad you need to mess up to accomplish something like that.

While trying to look this video up earlier I happened upon a video of an Israeli Blackhawk doing a similar rescue, but they had no litter. I'm not certain how they hooked the dude up but he was hanging below the two medics the entire ride up and slipped out and fell like 50 feet back down. Hella dead. Couldn't find any actual articles about that though.

The front quarter was open to the rafters but the back 3/4 had multiple hayloft levels. She used a rope in the rafters and jumped off the top level and was dangling out in space. Good 35-40 feet up. Had to call the local vollie FD to get her down. Naturally they drop her. Paperwork and assorted bullshit explaining the post mortem injuries to the medical examiner were probably the most involved of my entire EMS career. Tldr, that report probably isn't as interesting as you'd think.
