Very poorly designed, lazy cost cutting, sloppy work, negligence, incompetence. Normally countries do a better job designing their space rockets specifically to avoid this, it is very easy to avoid but it takes effort and work and you can apparently still get the rocket to go all the way into outer space and claim a 'win' and let the foreigner deal with the big metal thing falling out of the sky. I guess it's an improvement over their previous launch where it fell over and flew sideways into a village and exploded and killed god only knows how many people.
