No-prize: Red Skull has a latex face mask appliance so he can eat food without it falling out, smoke, articulate languages other than THE TRUE SKULLDEUTSCH more easily, and so on. He could have gotten one that would have looked more or less like a normal person, but as he is a Nazi dipshit, he instead got it to look also like a red skull.

If I remember right it's supposed to be two different eras of Red Skull. The left one for the story involving his parents and the right one for the present.

According to the synopsis, it's because the red skull that tried to recruit spidey's parents as agents is dead and someone else has since taken on his identity, apparently with a redesign of the mask.

Wasn't the Red Skull a clone of Steve Rogers with Johann Schmidt's brain inside (and of course, a red skull) for a while?

Red Skull stole Xavier’s brain for a while and threw it in his head to gain mutant powers. Or was it he moved his brain into Xavier’s body?

Anyways poo poo got hosed up and that’s how we ended with Red Onslaught.

Yeah. Immediately after Steve Rogers and John Walker had their decisive battle over who was Captain America, Red Skull attacked Steve, but his red mist of death got blown back into his face. So, Red Skull's Steve Rogers face got turned into a Red Skull face.

All the way up until he was vaporized in the Mark Waid run in 1994/5. He later returned in new bodies but I believe his new form was a construct and not the cloned body restored.

Incidentally Steve's serum was revitalized because of a blood transfusion from the Skull.

The Red Skull on the left (with Peter Parker's parents and unmasked Peter) is from a flashback that explains how Ma and Pa Parker didn't actually die as Cold War spies, but were recruited to be double agents by the Red Skull. Then as they were working as double agents the Soviets kidnapped them and tried to make them triple agents, but they refused and were stuck in a gulag all these years and just recently got released.

The Red Skull on the right is meant to be the present day Red Skull, who is Doing Thangs with Taskmaster in the present day in the same book.

1) Ma and Pa Parker were Life Model Decoys that were sent out to mentally break Spider-Man by Chameleon, who was later revealed to be manipulated into doing so by the Jackal, who was later revealed to be manipulated into doing so by Norman Osborn.
2) Flashback Red Skull by this point had already been revealed as a second imposter Red Skull, as they had established that real Skull (Johann Schmidt) went into suspended animation at the end of World War II and only got revived after Steve Rogers came back. There was a Red Skull in the 1950s Commie Smasher Captain America comics, and much like the retcon that established that the Cap/Bucky in those comics weren't Rogers/Barnes, there was a retcon that the Commie Red Skull was a different guy, who by the time that this Spider-Man comic came out had been murdered by the Real Red Skull. Commie Red Skull just wore a mask he found that WWII era Red Skull used.
3) The Real Red Skull at this point had his brain put into a clone of Steve Rogers. That clone body eventually got exposed to his Red Skull Dust or whatever, and so his face was an actual Red Skull, it wasn't a mask.

So it make sense that they'd look different, even if literally nothing else about the above does.

It also looks worse than it is because A tapers at the top whereas R is widens.

Left Skull is the commie imposter who killed Peter's parents. Right Skull is the original, who has since reclaimed the title.

This is literally just a portrait of a dude, but somehow it stopped me in my tracks. It's just... I mean, a 1977 funny book about punchmans looks like that. Sometimes the simplest stuff, done well, is what carries the most weight.
