There's also a degree to which hospital bills are a deliberate inflation to make insurance companies look better. So, like, you'll be billed $50k, your insurance company will say you own 2k and they'll pay the rest, and then they'll settle with the hospital for 10k. That way you think you're getting a great deal, and the hospital still gets paid.
