I've watched and participated in police active shooter training. A couple of pretend bad guys were running around an empty building with starter pistols, shooting blanks. The cops entered in small groups, and started clearing the building. The cops weren't given any weapons (lucky for everyone), but were instructed that if they wanted to shoot a target, to do a finger gun and yell BANG.

While watching the cops clear the building, it became very clear that if this had been a real scenario, there would have been a LOT of friendly fire. Several times, 2 groups of cops would come around a corner and bump into each other. Then they'd point their finger guns at each other screaming orders for a couple seconds before realizing "Oh wait, you guys are cops too".
The instructor running the scenario had to yell at a few of the cops to take off their sunglasses while inside. "I don't care how cool you think you look, take off your sunglasses, idiot!"

Another training scenario involved 2 bad guys in a large conference room with several hostages (of which I was one). Cops came in to the room in groups of 3 to try and free the hostages. First cop comes in, and sees one of the bad guys with his back to the door, holding an AR-15 by the top handle. Cop #1 orders the bad guy to drop the weapon. The bad guy starts to lower the gun to the ground, when Cop #2 barrels in, yelling BANG BANG BANG BANG and lights the guy up.
The other bad guy kills the hostages (I got to play dead several times thanks to their gently caress-ups), and takes a few cops down before being shot as well. When doing a debrief of the scenario, the instructor asked Cop #2 why he chose to go in blasting, when Cop #1 had already ordered the bad guy to drop his weapon. Cop #2 answered "He wasn't dropping it fast enough."

If I were ever in a real active shooter situation, I'd be just as worried about the cops killing me.

The first time a cadet pointed a training gun at her face it was a bit shocking but at least it was an instant fail for the guy.

Nope- the Police chief in charge took over and did nothing. The Border Patrol eventually yelled out "anyone alive in there?" "Yes!" "BANG". "Ok we go in now".
