not an expert as I was a child at the time but its my understanding that it was initially a medical term, with similar origin to 'idiot' or 'moron'. which also came to be a general insult meaning low intelligence. unsure why this one in particular started being regarded as so offensive, probably just because now is when basically everyone is online + talking to each other I guess
