Sometimes it's necessary to weld the manhole cover to the collar, for example if that location has frequent surcharging where the stormwater flow from below comes out the top of the manhole stack. We try to avoid that when designing a stormwater system, but sometimes you had bad assumptions, or drainage areas change, and it's better to make a hassle for the maintenance guys rather than creating severe hazards when the manhole slides away in a storm. Generally, though, it's only welded in a few spots so that you don't have to rip the entire collar off to access the manhole later.
