yeah im not sure. Seem like there's a lot of wiggle room for what that could mean in terms of his long-term recovery, but at this point i'll obviously take him not being brain dead as massively good news.

You can't be awake and intubated if you have a gag reflex.

Not an ER doc, but I assume they took the tube out once it was clear he wasn't a risk to need resuscitated again and then tapered off the sedation to let him wake up

It makes sense. I think it just means showing signs of responsiveness and essentially... *not* brain death.

When my sister passed away she was put into a coma for intubation (emergency dialysis) and when they tried to raise her out of the coma, she was not "neurologically intact." There was no responsiveness or anything to suggest her brain was functioning.

Maybe she was paralyzed by drugs somehow, but when my Grandmother went on a vent (due to a blood infection that ultimately killed her), she was awake and mouthing words around the vent tube for a couple of days before she finally slipped into a coma.

Neuro intact means that they’ve evaluated him for sensorimotor deficits and prob did brain MRI to evaluate for anoxic brain injury.

You can be “awake” enough to open your eyes with tube in place and there’s a scale to evaluate level of sedation (RASS). Patients aren’t generally kept -5, I think generally -1 is good for neurological assessment from what I remember from ICU rotation.

It's still very possible he has neurological damage in other regions aside from the pons & medulla, and it may take him a very long time to recover, and he may never fully recover in some ways.

Or hell, maybe he will. It's great news that he's apparently able to breathe on his own and responds to stimuli, but he's not out of the woods yet.

(I think I'm writing this mostly to temper my own raised expectations, I'm really not trying to be a wet blanket.)

Yeah, -1 to -2 is typically where you want them, the sweet spot where they can wake up when prompted and follow commands but not necessarily be bucking the vent/generally uncomfortable.

For a question that was asked above, you do generally hold sedation before extubation, with some rare exceptions (such as extubating off of a ver low dose of fentanyl). You do this because you want to give them the best chance at not being reintubated

I worked ICU for 10 years doing exactly what is going on with him right now.

Since he’s past the normal post cardiac arrest protocols, you lighten sedation and look for responses like eye opening or hand grasps. Then if their still synching with the vent you continue to decrease the sedation and hope they can do more meaningful interaction like nodding or writing.

However, since it appears he suffered a lung injury you need to keep him as synched with the vent as he heals.

Once he’s good you can try completely turning off sedation and putting his vent into a mode where he breathes on his own. If he gets tired you put the stuff back on and do it again later like a workout.

Tube comes out when the doctor seems it worth the risk or the nurse isn’t there and the patient removes it themselves. Some doctors I’ve worked with were cowards and had to be 100% sure the patient would be fine and would let them breath for like 1-2 hours as they do rounds. That is very frustrating as a nurse cause that patient is very much stressed out since they’re breathing thru a straw.
