fentanyl had already been a problem in the US, it just reached a point that it wasn't something you were paying attention to/reached the critical mass to be a news story around then. The 2020s is when it probably got on your radar since that's when drugs not associated with opioid use were increasingly cut with it, and also cops started blaming their panic attacks on it. Not to mentioned we got to point to bad actors like insys and their rapping fentanyl mascot.  Way easier to scapegoat a bad actor rather than the sickness of the system grinding up people while also looking for new products to sell workers who had been mangled as productivity had been squeezed out of them. The ramping up of the US war on drugs during the presidency of two demented grampas also shifted the calculus around smuggling and distribution; why risk a cargo container of one substance imported from warzones when a briefcase from a lab could do the trick?
