Dictionaries aren't prescriptive ("you can only use words that are in them"), they're descriptive (documenting what words people use). They're always going to lag behind actual usage to a greater or lesser extent, maybe a few months, maybe a few years, maybe a decade, maybe a century for loanwords. So, the answer to "but what did people say before then?" is that they used the same word, it just wasn't in the dictionary.

That's a common explanation but the best answer is "probably at least in some cases". The kippah is quite old and throughout the times it's been in and out of style among various groups for various reasons (though the recent popularity is probably the high water mark by a lot). Honestly the 19th century revival probably had as much to do with decreasing hat prices bringing secular hats back into style as anything else. The point is to always cover your head as a sort of nod to God, and the kippah is much more discreet under a secular hat than a scholar's cap or skullcap would be. As far as being discrete, all the Jews I've known have largely agreed that, as is usual for the faith, God Gets It, as in He knows what's up and if you take it off to avoid discrimination that's cool, even if it's a lot. Also worth noting that the point is normally regarded to be the head covering, not the Kippah specifically, so you can just wear hoodies or baseball caps or whatever and it's cool. Having a safety kippah under allows you to take the hat off, but it's not strictly necessary. Also if anyone says it's not sufficient or somehow disrespectful it is extremely literally your god-given directive to (respectfully) argue with them, as He is never happier than when His children get all rules lawyery over His word. Bear in mind that I came up in South Florida which is kind of a melting pot and tends to have the most permissive practices. Other people might feel more strongly in other places. In France the Académie Française is the authoritative and explicitly prescriptive keepers of the language, and it is a crime to use unrecognized words in some contexts.
