The Troy McClure fish thing was easily the most confused I was by a joke on the show as a kid. I remember asking my mom about it and she laughed without answering.

To be fair they did pick specifically the most absurd thing they could think of and treated it vaguely. That episode hits pretty different nowadays, with Troy being so clearly uncomfortable and distressed trying to consummate his marriage that he's clearly just not capable of or interested in, especially since the episode treats him fairly sympathetically.
