For a while, he didn't. There's a few episodes where Giovanni assigns jobs to Jessie and James that he actively hopes will get them killed, like the one where he has them pilot an airship, then gloats that the only thing he's maintained on them in years is their insurance policies. I think for the entirety of Hoenn at least they've actually been fired, but don't realise it.

It does swell a bit! The body is meant to be working under Earth's gravity, so when you're in zero/microgravity the heart's pumping a bit more blood to the upper body than is meant to be there, leading to a bit of puffiness and reddening especially in the head
