Adding to everyone else's response: its origin came as a coping mechanism when the narrative of "clinton lost because of racism" started giving way to "clinton lost because she didnt appeal to midwest issues". Basically by joking about it whenever racist statements came up, one could soothe the loss of midwest stronghold states by denying the emerging narrative with anecdotal statements. It started being used in earnest when it was revealed obama voters (who voted for a black man) didn't turn out to vote.

For every person that didn't vote for Clinton because they were worried about their jobs, there was a Jimmy Ray Bob Sue Cletus that didn't vote for her because she wasn't going to put the Mexicans in their place.

So if Bill hadn't encouraged Trump to run, and Hillary hadn't encouraged the media to treat him like a real candidate she might have won, yes.

that's not really accurate. the dnc (distinct from hfa) wanted the media to cover candidates like ben carson, trump, etc as the face of the republican party. that's what they meant by "pied piper" strategy - it was meant to tarnish the gop brand in general, and it was a good idea (and not a new one, either). it's probably no surprise the dems would rather face trump than jeb in the general election, but at the end of the day trump won because the gop consultant class collectively underestimated him, because the field was too crowded, and because that's what republican voters wanted.
