It's a lot more than just a normal NG+ with a bunch of new items as you progress through the tiers, a secret third path to the ants Infected which affects your next NG+ tier and has its own special drops, more weapon upgrades (not armour, sadly) and special weapons and trinket drops that can roll random* powerful stats. *Pre-set list. There's also some cosmetic changes and enemy placement change, like t3s in the bottom yard. You don't lose your base, which is nice, but most everything else on the map resets.
