Immediately after Steve Rogers and John Walker had their decisive battle over who was Captain America, Red Skull attacked Steve, but his red mist of death got blown back into his face. So, Red Skull's Steve Rogers face got turned into a Red Skull face.

All the way up until he was vaporized in the Mark Waid run in 1994/5. He later returned in new bodies but I believe his new form was a construct and not the cloned body restored. Incidentally Steve's serum was revitalized because of a blood transfusion from the Skull.

site posted:why is red skulls face changing here. is he switching masks depending on whether Peter is suited up The Red Skull on the right is meant to be the present day Red Skull, who is Doing Thangs with Taskmaster in the present day in the same book. 1) Ma and Pa Parker were Life Model Decoys that were sent out to mentally break Spider-Man by Chameleon, who was later revealed to be manipulated into doing so by the Jackal, who was later revealed to be manipulated into doing so by Norman Osborn. 2) Flashback Red Skull by this point had already been revealed as a second imposter Red Skull, as they had established that real Skull (Johann Schmidt) went into suspended animation at the end of World War II and only got revived after Steve Rogers came back. There was a Red Skull in the 1950s Commie Smasher Captain America comics, and much like the retcon that established that the Cap/Bucky in those comics weren't Rogers/Barnes, there was a retcon that the Commie Red Skull was a different guy, who by the time that this Spider-Man comic came out had been murdered by the Real Red Skull. Commie Red Skull just wore a mask he found that WWII era Red Skull used. 3) The Real Red Skull at this point had his brain put into a clone of Steve Rogers. That clone body eventually got exposed to his Red Skull Dust or whatever, and so his face was an actual Red Skull, it wasn't a mask. So it make sense that they'd look different, even if literally nothing else about the above does.
