That blond kid who turned out to be Ultimate Wolverine's son towards the last couple of years of Ultimate X-Men is in the 616 somewhere. I seem to remember he was a reasonably big part of Cullen Bunn's book with the teenage versions of the original 5.

Al Ewing also had the Maker bring back Ultimate Tony, Hulk, Hank, and Jan in the last couple of issues of Ultimates 2 a couple of years ago. Ultimate Steve got killed along the way.

I don't think he's been seen since that whole ridiculous Venomverse event. Which is probably for the best since he was still infected with one of those symbiote eating creatures. He would've been pretty useful during Ultimate Carnage at least.

It would have been funny to see him mowing through Carnages like loving Pac-Man while Spider-Man and company were having such a hard time

The Wikipedia article for Brainiac states it's a portmanteau of "brain" and "maniac", but I'd bet a box of donuts it was also influenced by the ENIAC computer

the entire issue is just that exact same frame just with different times and different heroes getting punched. there's no context and it's never brought up again.
