Initially you write one statement to be evaluated and everything works fine. Then you have to make a decision if the counter has some other value. So you say if it's value I definitely need to do it this way and if it's value 2 I may need to do it a different way. Then all the sudden the code is done and you realize that you are in essence doing the same thing regardless of value. At this point you may or may not have time to fix it. So I think it's a lack of pre-planning.
