It was a fairly simple roguelike for Windows that used icons as its graphical elements.

As far as the "solve math problems to open doors" gameplay goes, it might also have been OutNumbered - the setting was a mad-sciencey TV station. Listen to this - if a PC speaker version of the main theme is burned into your brain, you've played OutNumbered.
