For removing difficult hooks, the most versatile tool is a pair of fishing forceps like these. The biggest help though is removing the barbs from your hooks. You can use the forceps to press down on the barb and fold it flat against the hook, or break it off entirely. It's been my experience that the barbs aren't necessary if you're careful to keep a bend on your rod and tension on the line, which you should be anyway.
