My guess is they had both actors just record every line for simplicity's sake. And then they put it in the game to prevent a crash from the game trying to reach a voice file that doesn't exist.

That glitch also happens in TF2 sometimes.

Though it doesn't give you infinite ammo, it just plays the reloading animation until you fire or switch weapons.

Mario can pass through enemies because the hitbox is smaller then the Mario sprite.

Tool Assisted Runs are typically done by a player making inputs on an emulator with the game artificially slowed down drastically, often for a single frame at a time. A record of these inputs is then saved and played in real time.
