Don't know for the first 2, but for the third:

Holy sites for all unorganized religions are a mix of more local holy areas and sites that would be considered supremely important conquests, if they were to happen. The idea is gameplay mechanic related. To reform your religion and become the new head of the religion, you're going to need to either have a united religious homeland or have some important areas at home and then do some crazy conquering and build victory temples at some important locations.

What are the advantages of accepting a cultural shift from Norse to Norwegian, etc.? I have only a single Norwegian province, the rest are all Norse, is it just that they will start becoming Norwegian and I will eventually need to to as well to remain the same culture as my provinces?

There are a number of good portages in the Russian and Belorussian river systems. That's how the Vikings were able to do that exact route in real life, and how they pillaged Iran and Azerbaijan as well.
