Backing up the perineum theory. When she goes to her doctor I would recommend specifically asking about the area and seeing if she gets swelling/pain from the obgyn's light manipulation of the area with a qtip or whatever they use.

Try this yourself as well or have her try it herself to check (lightly touch the back end of the vagina vicinity of the perineum and note when/where / if pain happens.

Disclaimer - I am not a woman myself but I have encountered this issue before.
