Battletoads  Specifically, level 3: https://www.youtube.com/watch?v=ye-kPP3D27o&t=49s I could never pass this level as a kid, and looking at playthroughs now I can see I always died juuust before the end of the level. Probably a good thing, the later levels look even harder
