If you're ok with doing a factory reset, you can always put KitKat back on it and not update it back to Lollipop for awhile. Then you can be more confident that it is or is not a hardware problem.

Most apps that open media will automatically scan your storage and show any media they can open, but without knowing exactly what you're trying to do, I can't say if it will work exactly how you want.

Without knowing more about what he did or if his phone is rooted/running some random custom rom, it's tough to say. You can imagine if he did something like changed the default install path for apps or used some hacky app to link the internal memory and SD card, you could get problems like this.
