Its not just 'super fat' women. Your first pregnancy you don't really show much until about the 5th-6th month for some women, even stick-thin ones, and if you have few or none of the stereotypical early pregnancy symptoms like morning sickness you can easily write the less common ones off as stress (fatigue, weight gain, loss of appetite, missing period). Combine that with denial and it can easily go past 12 weeks without a clue.
