You order the table by nationality, so Scotland, Ireland, Wales, Italy, SA. Then you alternate the top team of each nation between conferences, the second team goes in the other conference. If they're a four team nation then the third team goes with the second and the fourth team stay with the first to balance out that nation's strength. I think that works.
