I don't think anyone is trying to pass it off as that. It's not a surgeon map. Healthcare networks employ tons of people. Same with walmart. They don't have thousands of CEOs.

Actually being part of the city/state government? A lot of places get state/federal funds on some level, it doesn't make them all government entities.
