Actually the failure is in a lack of community and/or government structures to ensure it either doesn't happen or that communities at risk are prepared.

FYI I know it's a bit late, but the percentage increase of those emissions over 700% is a bullshit number and there's no way it's defensible if there's not a similar increase in output from the facility. All it proves is that they're not taking the disclosure seriously, and if they think they are, they can show their working.
