Cena was the one doing the hip toss. http://www.wwe.com/videos/john-cena-suffers-injury-against-mr-kennedy-raw-october-1-2007 It doesn't look obvious what caused it.

He had some events mixed up. Kennedy actually wasn't fired until May 2009. He doesn't seem to have been especially punished for the Cena injury. He went into a Jeff Hardy feud, then HBK. It was after nearly injuring Orton that he got fired. According to Kennedy himself, Orton was pissed and went to Vince and had Cena back him up.
