I haven't played too much but I've run into a fair number of monsters that have taken next-to-no damage from my attacks and instakilled me, so it is truly in that Gothic vein. Still weighing faction choices and gingerly exploring / hoarding herbs.

Some of the guns have alt-mode shots that do crowd control stuff, and you want to get good at dodging while aiming for things that beeline straight for your face. I have not, however, finished the game, so I'm not sure if there's a stark drop-off.
