it's from an old flash game (similar to plague inc) where you tried to infect the whole world w/ a disease. madagascar was notoriously hard to infect b/c the minute it their officials would sniff any sort of pandemic they'd close ports and that was that.

you build research stations, work to treat the disease where you can to prevent outbreaks, and work on a cure before you're overwhelmed. really depends on how well you can share knowledge and where the initial outbreaks occur.
