States and localities have departments of health that can still enforce local containment measures and will likely do so more and more at their own discretion as it's obvious that the federal response is not going to be there for them. Even without proper testing it's not like this type of scenario of disease has never been considered.

200k americans dying would be slightly worse than a bad flu season, so it's not some society collapsing event yet.
