Not from gacha stuff, but I have had plenty of only games accidentally wipe stuff that should be stored server side require client side input. Specifically I have seen more or less the exact scene from Odd Taxi occur in Mechwarrior Online due to an internet connection drop. Hell I've seen it duplicate reward crates due to bad internet connections too. I also recall people complaining about this with limited time Dead by Daylight cosmetics not properly being associated to their account. The game being poorly programmed isn't really a plot hole.
